A bit of a random one that falls under the "I crap you not" category, but it seems the combination of slippery glass back + a wireless charger (non-Samsung brand) that has a high pitch whine seems to be just enough to cause the phone to slowly rotate until it completely falls off the charger after about 5 minutes.
Just throwing it out there in case I'm not the only one. I'm going to be putting a Ringke slim case on it in a couple days like I did with the S3, S4, and S5 which will solve the problem.
